因晚期喉癌行全喉切除术、创伤或其他疾病而失去喉部的患者,不仅是失去了正常的喉部结构、气管通气及发声功能,同时其嗅觉及味觉功能也受到一定程度的损害,并且增加了气管支气管感染的风险,从而严重影响了患者的呼吸、吞咽及与人沟通能力。以往,此类患者只能通过使用人工喉或通过其他方法来恢复讲话和呼吸的能力。但是,这些方法均存在一定的限制——如音质不佳、需要定期更换等。

In the past, such patients could only restore their ability to speak and breathe through the use of an artificial larynx or other methods. However, these methods have certain limitations, such as poor sound quality and the need for regular replacement.

With the continuous progress and innovation of medical technology, laryngeal transplantation has brought hope to patients who have lost their larynx—by transplanting healthy laryngeal tissues and organs, restoring the patients' laryngeal function and voice, and improving their quality of life. It can also provide a new treatment method for serious diseases such as laryngeal cancer, increasing the patients' chances of survival. Laryngeal transplantation is one of the proud specialties of the Department of Otolaryngology-Head and Neck Surgery at West China Hospital.
然而,喉移植手术目前仍处于发展阶段,尚未被广泛应用于临床实践中。至目前,全球只有少数几例成功的喉移植手术案例。其中,最著名的是1998年由美国的Marshall Strome, M.D.及其研究团队完成的一例喉移植手术,也是全球第一例成功的喉移植手术。此后,其他欧美国家也陆续开展此类手术,然而,由于喉移植手术的复杂及困难程度,经查阅文献;迄今为止仅有三例详细报道的喉移植成功病例(美国2例;芬兰1例),且亚洲范围暂无开展喉移植手术的医疗团队。
However, laryngeal transplantation is still in the developmental stage and has not yet been widely applied in clinical practice. To date, there have been only a few successful cases of laryngeal transplantation worldwide. Among them, the most famous is the first successful laryngeal transplantation performed by Dr. Marshall Strome, M.D. and his research team in the United States in 1998. Since then, similar surgeries have been conducted in other Western countries. So far, there have only been three detailed reported successful cases of laryngeal transplantation (two in the United States and one in Finland) due to the complexity and difficulty of the procedure, and no medical teams in Asia have conducted laryngeal transplantation.

There are many technical and risk-related challenges associated with laryngeal transplantation. Firstly, laryngeal transplantation requires a suitable donor, and the success rate of matching is low, while the number of donors is limited. Secondly, the procedure is highly complex and risky, requiring a highly specialized medical team and precise surgical equipment. Additionally, the transplant recipient needs to receive long-term immunosuppressive treatment to prevent rejection.
“喉移植手术是每一位头颈外科医生的梦想,相当于珠穆朗玛峰一样的位置。”喉移植团队创始人陈飞教授觉得,既然国外可以做,就以他们为参照,一步一步地走,一个问题一个问题地攻克。“开始没想过会成功,大家也都认为我们不会成功。当时就觉得即使失败,在探索的过程中能掌握一些显微外科技术、神经和血管的修复重建技术等头颈部修复的尖端技术,也是有很大的科研价值。”
"Laryngeal transplantation is the dream of every head and neck surgeon, equivalent to the summit of Mount Everest." Professor Chen Fei, founder of the laryngeal transplant team, believes that since foreign countries can do it, they should take them as a reference and take step-by-step actions, addressing challenges one by one. "At the beginning, we didn't think we would succeed. Everyone also thought we wouldn't succeed. At that time, I felt that even if it failed, it would still hold significant scientific value to master some cutting-edge technologies in head and neck repair, such as microsurgical techniques and nerve and blood vessel repair and reconstruction, during the process of exploration."
为了开展全喉移植手术,陈飞及其团队从2006年就开始基础研究,从实验用的老鼠、兔子开始,先做自体喉移植,又尝试异体喉移植。“这17年确实比较辛苦,也遇到过困难,全靠情感和梦想支撑。”陈飞表示,常规院内手术也加速了技术经验的积累,“我们重点开展了头颈部重建修复手术,每年自己主刀上百台游离皮瓣手术,还有喉返神经损伤的修复。”
In order to perform a total laryngeal transplantation, Chen Fei and his team began basic research in 2006, starting with experiments on mice and rabbits. They first performed autologous laryngeal transplants and then tried allogeneic laryngeal transplants. "The past 17 years have been quite tough, and we've encountered difficulties. But it was all driven by emotion and dreams," Chen Fei said. Conventional surgeries also accelerated the accumulation of technical experience. "We focused on head and neck reconstruction and repair surgeries, performing over 100 free flap surgeries each year, as well as the repair of recurrent laryngeal nerve injury."
